The Family of the late Hazel Isabel Maltby (OAM) wish to sincerely thank Dr David Cairncross and caring staff of the Bowen Medical Centre, doctors and staff of the Bowen Hospital, Blue Care Bowen, HAAC ladies and Bishop Bill Ray Anglican Bishop of North Queensland for their dedicated care and attention to Hazel.

Special thanks also to the Holy Trinity Bowen Anglican ladies and gentleman, Bowen Independent, Jochheim's Pie Shop and Whitsunday Funerals for their contribution.

The many family members, relatives and friends some of whom travelled long distances to be present and for the support of those who sent floral tributes, donations and message of sympathy to our families during the time of our sad loss.
