Passed away peacefully at home 18th May surrounded by love.

Dearly loved son of Catherine and Wayne, loved brother of Brenton and Stefan, loved nephew, cousin and dear friend to many.

A very special soul who lifted spirits with his infectious smile, humour and empathetic nature.

Jays strength, courage and determination during his epic battle, was testament to who he was.

Forever in our hearts, his memory will live on through the love, laughter and joy he brought into our lives.
