Family and Friends are invited to attend a Celebration of Jake's Life, to be held at Elevation Church, 56 Caloola Drive, Tweed Heads, on Friday, 8th April, 2016, at 11 a.m..A memorial service for Jake will be held in Sydney. Date, time and venue to be confirmed
